Children’s Wisconsin, Wisconsin’s only independent healthcare system dedicated solely to pediatric care, has partnered with Xealth and Locus Health to remotely monitor pediatric patients following hospital discharge. With an initial focus on cardiology, enteral feeding, and patients with tracheostomy vents, the program allows care teams to track patients from home, enabling quicker interventions and better outcomes while reducing the burden of extended hospital stays on children and their families. The solution is integrated directly into the electronic health record, giving clinicians seamless access to patient data within their existing workflows. Early results have already shown promising outcomes, including avoided emergency room visits and faster weaning from ventilators and feeding tubes.
